Biography

Mr. Farhad Khosravi, also known as Fred, MS co-founded Maya Medical, Inc. in 2010. Mr. Khosravi is a Co-Founder at Incept, LLC and serves as its Managing Director, Managing Partner and Managing Member. He Co-Founded Access Closure, Inc. in 2002. He serves as the Chairman, President and Chief Executive Officer of Ostial Corporation. He co-founded Sadra Medical, Inc. in 2003 and has been its Chairman since October 2008 and also serves as its Chief Executive Officer. He is Co-founder of Confluent Surgical, Inc., MarketRx, Inc., Ostial Corporation., Occular Therapeutix, Inc., HotSpur, Inc., Nextplane, Inc., and Claret Medical, Inc. He is a Co-Founder of SquareOne and serves as its Chairman. He is a Silicon Valley medical device entrepreneur. He has 25 years of experience in the medical device business and held key management positions with Focal, Inc., Guidant Corporation and Alcon Surgical, Inc. He served as the Chief Executive Officer and President of Access Closure, Inc. from 2002 to June 2010, which he co-founded in 2002. He co-founded and served as the President at Embolic Protection, Inc. (EPI). Prior to Embolic Protection Inc. He founded EndoTex Interventional Systems, where he served as the Chief Executive Officer from December 1997 and Chairman from 1999 and led the team that developed minimally invasive solutions for treatment of carotid artery disease. He co-founded Ocular Therapeutix, Inc. He has been the Chairman of Hotspur Technologies, Inc since June 14, 2011, which he also co-founded. He serves as Chairman of the Board of Claret Medical, Inc. He serves as the Chairman of the board at Nextplane, Inc. He has been Chairman of Access Closure, Inc. since June 2010. Mr. Khosravi served as the Chairman of the Board of Directors of Confluent Surgical, Inc. He serves as a Trustee of Center for Strategic and International Studies, Inc. (CSIS). He serves as Director of Augmenix, Inc., Sadra Medical, Inc. and Access Closure, Inc. He served as a Director of Ocular Therapeutix, Inc. until June 2013. He serves as a Board Adviser of EmboMedics, Inc. He served as Director of Marketrx, Inc., and Kerberos Proximal Solutions. He served as a Director of Hotspur. He also served on the Board of Advanced Stent Technologies. He is the author or coauthor on over 105 issued or filed patent applications, involving unique and novel medical devices. Mr. Khosravi holds a B.S. degree in Mechanical Engineering, summa cum laude and a Master of Science degree in Mechanical Engineering, both from Tennessee Technological University.
