Biography
Mr . Al Hammonds Serves as Executive Director, Millennium Collaborative Care holds responsibility for Lead and coordinate projects toward successful completion of DSRIP Project metrics and milestones , Organize and implement clinical redesign initiatives related to DSRIP projects in collaboration with health care providers and community based organizations throughout 8 county region of WNY and Compile, analyze, evaluate, and review operational documents such as program reports, data, and financial reports . He is having Licence on Certified Six Sigma Black Belt . He won awards as Black Western NY Feature Article (2015), American Society of Quality (ASQ) Awards (2008 - 2010) and UB Service Award /Graduate School Awareness Conf. (2008) . Having 32 years experience in Administration / Management . He holds Bachelor’s Degree in Industrial Management/Industrial Engineering – Purdue University Health Care Executive Program Graduate Degree – UCLA
